The role

As Care Coordinator, you will help keep our care service running smoothly. You will work closely with our Clients, their families and our team of Carers, balancing excellent customer service with effective coordination and attention to detail.

Your responsibilities will include:

• Coordinating Carer rotas and ensuring care visits are covered effectively

• Managing day-to-day changes including sickness, annual leave and changes in Client needs

• Supporting continuity of care and effective matching of Carers and Clients

• Coordinating Carer supervisions, spot checks, competencies and training

• Supporting recruitment, interviews, onboarding and compliance checks

• Monitoring training and employee records to ensure requirements remain up to date

• Supporting Client assessments, reviews and person-centred care planning

• Responding professionally to new care enquiries and supporting prospective Clients and their families

• Maintaining accurate records and supporting our ongoing CQC compliance and quality standards

About you

We are looking for someone who is organised, confident and able to build positive relationships with a wide range of people.

You will ideally have:

• Experience within domiciliary care, care coordination or a similar role

• Excellent organisation and time-management skills

• Strong communication and relationship-building skills

• Confidence managing competing priorities in a busy environment

• Good IT skills; experience using care management or scheduling systems would be an advantage

• A good understanding of person-centred care and quality standards

• A relevant Health and Social Care qualification would be desirable

You must hold a full UK/EU driving licence, have daily access to a car with appropriate business insurance and have the right to work in the UK.
